Lillian Levine, is a certified T-Trainer of the Stanford model of evidence-based health promotion programs, Chronic Disease Self-management and Diabetes Self-management programs. She has trained hundreds of Master Trainers and program leaders during her 6 years of service delivering health self-management programs across Ohio. Lillian has made a personal investment in improving the lives of individuals with chronic health conditions and their families. She also serves in the community at The Gathering Place, the Cleveland Orchestra and Fairhill Partners. Lillian has 20+ years in the field of health, science, technology, and education.
Stacey Rokoff, MSSA, LSW, has 12+ years leading health, wellness and prevention programming support the aging and family caregiving populations. She's impacted the transformation of health behaviors and disease management in thousands of individuals. Stacey is a certified T-Trainer of Stanford’s evidence-based self-management programs and Master Trainer of MaineHealth’s A Matter of Balance. She serves as adjunct faculty for the Mandel School of Applied Social Sciences, Case Western Reserve University and is an active member of Council on Older Persons (COOP). Stacey previously served on the Professional Advisory Board at the Alzheimer’s Association.
Helen M. Dumski, MA, RD, LD, served the Diabetes Partnership of Cleveland for more than a decade as President and CEO and Chief Operating Officer, managing a multi-million dollar operating budget and endowment, a 45 member Board of Directors, community and professional education, research, advocacy. Helen served as President of the Ohio Dietetic Association, was named the Ohio Diabetes Educator of the Year from the American Diabetes Association and was the recipient of the Trustee of the Year Award from the Diabetes Association of Greater Cleveland.

Mary Jayne Reedy, MBA,founder and President, leads a unique organization that links healthcare to evidence-based community programs. Her diverse experience in managed care, healthcare providers, non-profit, pharmaceutical, Medicaid and Medicare provides a wide perspective to many facets of patient-centered care transformation. Mary Jayne led market growth for Bristol-Myers Squibb, Alcon Laboratories, Human Arc and the American Red Cross. Currently she serves the American Red Cross Northern Ohio Region Blood Services on the Board of directors and earlier the Northeast Ohio Healthcare Financial Management Association. Certified Master Trainer by Stanford in self-management programs.
